Population Health Academy: Addressing Behavioral Health in Population Health Management


Barry J. Jacobs, Psy.D., a clinical psychologist, Director of Behavioral Sciences for the Crozer-Keystone Family Medicine Residency Program, and former board member of the Collaborative Family Healthcare Association will discuss 3 models of integrated primary care practice, including their staffing, incorporation into office flow, communication strategies, and means of reimbursement. Case illustrations will be provided of both the treatment of behavioral health conditions in primary care and the use of primary-care-based mental health clinicians to assist with patient activation and behavioral change.
