Population Health Academy: Community Based Care Management Interventions

Len Terranova, MS, MBA, Director of Planning and Performance, and Jason Turi, RN, MS, Associate Clinical Director, both with the Camden Coalition of Health- care Providers will be sharing the Coalition’s model for multi-disciplinary community based care management interventions, focused on connecting patients to existing health, social services and community resources. The intervention is driven by principles of shared decision making and care planning on bio-psycho social domains and is designed to coach patients to self-efficacy.
