Population Health Academy: Experience From a Population Health Internship: Transitions in Care and Behavioral Influence on Care Management


Jillian Gansert, BA, second-year Medical Student at the Cooper Medical School of Rowan University will share her 2014 summer research position with Cooper’s Center for Population Health. The research focused on the “Effects of Patient Engagement on Health and Usage in Patient Centered Medical Home”, exploring the connection between patient involvement in their medical care at a patient centered medical home and how it affects health outcomes and utilization.

Jennifer Bleznak, BS, member of the charter class of Cooper Medical School of Rowan University, worked at AtlantiCare Hospital in 2013 with Dr. Anthony Macchiavelli assessing various factors that influence 30-day hospital readmissions. Jennifer’s study used the parameters of Project BOOST, a program developed by the Society of Hospital Medicine that aims to identify consistent triggers for hospital readmissions that can be recognized and addressed with patients to improve discharge outcomes, to assess readmissions at AtlantiCare prior to its implementation of BOOST protocols. The goal of her study was to determine whether addressing these criteria could have influenced discharge success.
