The Diabetes Weigh
This five-part Diabetes Self-Management education program is recognized by the American Diabetes Association for excellence in education. It is designed to give you the skills needed to manage diabetes and reduce your risks for complications. In the first class, you will meet with an experienced diabetes educator to develop an individualized diabetes care plan and a personalized strategy to reach you goals in diabetes care. You will learn how food, medication, and exercise affect blood glucose levels. We also work closely with your primary care physician and endocrinologist to meet your health and lifestyle goals. This will help you develop confidence in the day-to-day decision making for a healthier lifestyle.
There are five series remaining in 2021:
The first series begins on Monday, September 13, 2021 and ends on October 4, 2021.
The second series begins on Thursday, September 16, 2021 and ends on October 14, 2021.
The third series begins Tuesday, October 25, 2021 and ends on November 22, 2021.
The fourth series begins Wednesday, October 26, 2021 and ends November 23, 2021.
Finally, the last series begins on Monday October 28, 2021 and ends on December 2, 2021.
This program is covered by most insurances and free to people with Cooper insurance.
In addition, we offer free monthly support groups for people who have graduated from The Diabetes Weigh program.
