Advance Care Planning: How to establish your goals

Join MD Anderson Cancer Center at Cooper expert, Lara Roseman MSN, APN-C for a free online information session discussing the services of Palliative Care for the cancer survivor. Lara will provide an overview of Advanced Care Planning including how to establish your goals of care, identifying a surrogate decision-maker, completing advanced directives, POLST (Physician Orders for Life-Sustaining Treatment), and right to die.

This one-hour program will include a Q&A session at the end of the presentation. We look forward to your participation.

Lara Roseman MSN, AGNP received her Bachelors in Science from Seton Hall University in South Orange, NJ and her Master in Science of Nursing in Adult Gerontology from Rutgers University. She worked as an oncology nurse in both outpatient chemotherapy infusion and on the inpatient oncology unit at Cooper University Health Care. She has been a member of the Palliative Medicine team at Cooper for the past 5 years and her special interests include pain and symptom management in the oncology population and advanced care planning.
