Transitioning Diabetes Care From Pediatric to Adult

Do you have a son or daughter with Type 1 diabetes who will be turning 18?  Or about to leave Pediatric Endocrinology? This means many changes; going to college or starting a job.  For young adults with diabetes it also means leaving pediatric care and transitioning to Adult Endocrinology.   Our team of experts in diabetes can help you and your son and daughter be prepared to make appointments, get labs, monitor and talk to a new team.  We can help you begin to have a caring discussion about leaving the diabetes team you have come to know and trust and transitioning to adult diabetes care. 

 In this free seminar, your pediatric diabetes educator, Brianna Marshall, the adult endocrine diabetes educator, Michelle Laranko, and an adult endocrinologist will offer tips to you and your son or daughter about this change.  As nurses with Type 1 diabetes, our educators bring years of experience into this discussion. In addition, Michelle has raised a son with Type 1 diabetes and offers compassion and care.    You will meet other families addressing this same transition. Parking is available.
